2022 Coaching Application (Hillsdale Athletes)

I attest that there have been no relevant charges against me since the attached PRC/VSS was conducted.  I further acknowledge that my PRC/VSS expires for the purposes of the Hillsdale Athletic Association on Dec 31st of the 2nd year from the date on the PRC (All PRCs dated in 2020 expire Dec 31 2022).
